File & Serve documents its own attendances and can provide service records based on what actually occurred. It does not select legal forms or give legal advice.
Factual record
Service Confirmation / Proof of Service
When an assignment or the included attempts are completed, File & Serve provides the applicable confirmation or proof record from its service history. The record should describe what File & Serve actually did and observed—not what a party hoped would happen.
Commissioned document
Affidavit when File & Serve completed service
Service Confirmation is included. For service performed by File & Serve, affidavit preparation ($25 per affidavit) plus independent commissioning ($25 per affidavit) is $50 + HST per affidavit; commissioning only is $25 + HST per affidavit when you supply a complete, unsigned affidavit for that same job. Commissioning only excludes preparation or correction. The actual server must review and swear or affirm the facts before a different authorized commissioner.
Client responsibility
The required proof depends on the proceeding
The client must determine whether an affidavit, certificate, confirmation, or another proof document is required, which form is correct, who must sign it, and where and when it must be filed. File & Serve does not select official forms or provide legal advice.
Dates, times, address, recipient identification, manner of service, attempts, and exhibits must match the factual record. A requested wording change cannot alter what actually occurred.

Is the affidavit automatically included?
Service Confirmation is included without an affidavit add-on. For service performed by File & Serve, choose preparation ($25 per affidavit) plus independent commissioning ($25 per affidavit) for $50 + HST per affidavit, or commissioning only for $25 + HST per affidavit when you supply a complete, unsigned affidavit for that same job. The actual server must review and swear or affirm the facts before a different authorized commissioner.
Can File & Serve choose my court form?
No. The client supplies the applicable form and instructions. We may prepare our actual server’s Affidavit of Service from the factual record of service performed by File & Serve. We do not select forms, provide legal advice, choose legal strategy or complete unrelated court forms.
Can an unsuccessful attempt be turned into successful service?
No. The record must accurately state what occurred.
Who decides whether the proof is legally sufficient?
The court or tribunal applies its rules. Clients should obtain legal advice when they are unsure what proof is required.

Affidavit quantities and commissioning only
Our standard online service prices one separate affidavit for each named service recipient in this order; the selected affidavit option applies to all listed recipients. Preparation is $25 and independent commissioning is $25 per affidavit, plus HST. Multiple pages, alternative addresses, visits or process servers do not multiply the affidavit quantity. Commissioning only requires a fully completed, unsigned form for each recipient after service; a blank form does not qualify. Different scopes or existing agreed prices must be confirmed with our office before payment. Accepted quotes and paid orders keep their agreed prices. This is our service-pricing policy, not a statement that every court requires separate affidavits in every case.
