Factual proof record

Commissioned affidavit based on completed service

When File & Serve completes service, the current request workflow offers an optional commissioned Affidavit of Service based on the company’s factual service record. The affidavit describes who attended, when, where and how service occurred.

File & Serve does not choose or complete official forms for the client

The affidavit add-on is limited to File & Serve’s own factual service record. We do not advise whether an affidavit is legally required, select a court form, revise client materials or make legal conclusions.
